Clients Rights

1. All clients’ information is confidential and shared with no one or no entity outside of Eradah without written consent signed by the client or guardian. This Release of Information (ROI) is available in the Customer Service Department at Eradah.
2. All clients’ information is kept private and limited to those on the client’s multidisciplinary team.
3. Clients and/or their guardians have the right to freedom from:
Abuse
Financial or any other form of exploitation
Retaliation
Humiliation
Neglect
4. Clients and/or their guardians have access to:
Information pertinent to the client promptly
Their own records
5. Clients and/or their guardians have the right to be informed of all aspects of their care, express choice, and the right to refuse any said care, including the following:
Service delivery
Release of information
Composition of their multidisciplinary team
Involvement in research
Concurrent services
6. Clients and/or their guardians have the right to investigate and resolve any alleged infringement upon these rights and use any legal counsel necessary according to the rights and laws of the Kingdom of Saudi Arabia.
7. Clients and/or their guardians have the right to access or referral to:
Legal entities for appropriate representation
Self-help support services
Advocacy support services
